Step 4: How do I apply for a Brake/Lamp station license?

To apply for a Brake and/or Lamp Station License, simply follow these 3 steps:

1. Fill out the application and sign it. You can download the application here. If you are unable to download the application, you can also request one by:
   1. Calling our Consumer Information Center to request the application at 1-800-952-5210.
   2. Visiting a local field office to pick up the application. Click here for a list of field offices.
2. Include a check or money order for the appropriate application fee. The fee for this license is $10 per license.
3. Mail the application, along with your fee, to the address on the application.

Warning: Your application must be submitted to the Department of Consumer Affairs (DCA) at the address listed on the application. An original signature and date are required. It is your responsibility to ensure that you complete the entire application and submit it with the correct fees. Submission of an incomplete or incorrect application will result in a delay in processing your application.
